We accept cash, credit cards [Visa, Mastercard, Discover, Debit cards, American Express]. Your out of pocket expenses are due at the time of your appointment.

We participate in most insurance plans. You will find a list of the most common accepted plans on this website.

Please contact your insurance company directly about getting your primary care doctor (PCP) changed to the doctor in our office. This can usually be done immediately or at the beginning of the next month, depending on the circumstances.

Since we handle most insurances, there is a good possibility you can. Call our office to verify we handle your new policy.

Whenever you are seeing a new doctor, you will need your insurance card and a list of medications you are currently taking. A list of previous doctors, including phone numbers, may also be needed to retrieve prior records.

Prescription refills are most easily attained by contacting your pharmacy where you originally filled the prescription. They can contact the office electronically which facilitates easier refills. Please note that the patient must be in good appointment standing with the physician for use of this method.

Call our office and set up an appointment after you contact your insurance company and advise them of the situation. They will give you a case number, claims adjuster name and number. Please bring this information with you on your first visit. Also, if you have retained an attorney, bring the attorney name, firm name and phone/fax numbers. If you haven’t retained these services, we may be able to give suggestions in this area. There is a form on this website you can download and fill out with the information which is needed. This will make it easier and less time consuming on your initial visit.

After your initial appointments, the doctor will decide if you have a “EMC”. This is a “Emergency Medical Condition”. This is the ability of the doctor to extend your coverage from the normal $2,500 to $10,000 worth of coverage. Remember, this coverage is by your auto insurance, not medical insurance. Every insurance policy in the state of Florida must have this coverage. This determination is made by extent of injuries and type of injuries.

Please contact the office within 24 hours of your appointment time.

You will receive a call the day before your appointment to confirm and remind. If you have a Monday appointment, this will take place on Friday.

I had been to a few chiropractors in the Trinity area and not one of them could relieve my lower back pain. All of them had tried to refer me to a pain management doctor. On advice from a friend, I went to see Dr. Greenfield and after a few mild adjustments to my back (not painful), and recommended physical therapy, mild lower back pain has been greatly reduced. Thanks to Dr. Greenfield my back feels better and without any prescription medications!!
